The state requires physicals for grades" Pre-K, K, 1, 3, 5, 7, 9, 11. Physicals are required for all new students and prior to extracurricular sports activities (grades 7- 12). The school provides the physicals. If you do not want the physical done at school, you must:

• Notify the school nurse in writing by October 1st and obtain a physical form.
• Have a physical done at your own expense and return the completed form to the school nurse by Dec. 1.

If the form is not returned, a physical will be done at school.
